 H1N1 Influenza Vaccination Clinic
for Monmouth County Residents
Where: Raritan High School
419 Middle Road,
Hazlet, NJ
When: Saturday, October 31, 2009
Noon to 4:00 pm
This clinic is ONLY for the following target groups:

1. Pregnant women
2. People who live with or care for children younger than 6 months of age,
3. Emergency Medical Services personnel,
4. Children 6 months through 4 years of age, and
5. Children 5 through 24 years of age who have chronic medical conditions.

Flumist only (noninjectable) is available for the following target groups:

1. 2 years – 24 years old with no chronic or acute illness
2. who live with or care for children younger than 6 months of age, with no chronic or acute illness

Target groups should consult their physician prior to receiving vaccine
Additional school‐based clinics for H1N1 vaccination will be held in the near future.
